Hospice care is patient-centered and patient-directed. It is designed to be an interdisciplinary team, dedicated to meet the needs of the patient and the family. Hospice providers prepare an individual plan of care based on the family's needs and goals; in consultation with the patient, his or her loved ones, the primary caregiver and the patient's physician. Direct care is then augmented by hospice and the care is provided wherever the patient calls home.
A hospice team consist of the patient and their primary caregiver/facility, the patient's personal physician, the Hospice Medical Director, Registered and Vocational Nurses, Social Worker, Chaplaincy or Spiritual Counselors, Trained Volunteers,and home health aides. Hospice can provide medications related to comfort and terminal diagnosis. As well as some needed supplies and durable medical equipment. Hospice is designed to support the care that the patient's primary caregiver provides.

We are dedicated to provide coordinated, comprehensive, and patient-centered care to address the physical, psychological,and spiritual pain associated with dying. All services, medications and durable medical equipment necessary for the comfort of the patient are included under insurance coverage
